Application Instructions

• The application deadline is 11:59 PM Eastern Standard Time on December 1, 2009.
• Applicants must apply online, using the application below.
• You may cut and paste information from other documents into this online application.
• Complete all parts of the application.
• Three letters of reference are required.
• Letters of reference must be sent via e-mail and must be received by December 1, 2009. Letters of reference should be sent to: fellows@ei.columbia.edu
• Applicants should send only the letters of reference. Anything sent other than letters of reference, i.e., publications, medical records, etc., will be discarded. Materials must be sent as either a Word document (file ends in ".doc") or an Adobe Acrobat document (file ends in ".pdf"). No JPEGS or other formats will be accepted.
• Please note that candidates should have all of the components of the application ready in a separate file before starting the online application. Every attempt to adjust or modify the application will result in re-submitting a complete new application.
• Referees who will be submitting reference letters on the applicant's behalf must include the applicant's name in the "subject line."
